Certain states have provided their residents, subject to various legal limitations and exceptions, with specific comprehensive privacy rights and/or privacy rights related to their personal health data.

At this time, we meet the applicable requirements of these states which mandate our compliance. This form is intended to be an all-inclusive form for California, Colorado, Virginia, Connecticut, Nevada, Washington, Oregon, Montana, Delaware, Iowa, New Hampshire and New Jersey residents to exercise their Right to Know. As privacy laws change, we will add more functionality and more covered residents.

For those residents of other states who do not afford their residents the same or similar privacy and/or health data rights, we encourage you to review our Privacy Policy to understand how we collect and use your personal information.

If you are a current resident of one of these states in which residents can exercise data privacy rights, or if you are a resident of Washington or Nevada or a resident of any state whose health data was collected in Washington or Nevada, you can use this form to make a Right to Know request for yourself or, if provided by applicable law, on behalf of another person.

There may be specific circumstances, where we may require additional information if we are unable to verify your request based on the information you provide, and we may contact you to request additional information to help us verify your request to begin processing your request.
